Services conforming to certification standard(s) as indicated by a Certification Mark;The certification mark, as used by persons authorized by the certifier, certifies that the medication therapy management service provided person has met the following standards of expertise in the field of medication therapy management: 1. Experience and Training: 2 years of MTM experience OR NBMTM-approved training OR degree in medication therapy management. 2. Licensure: Hold an unrestricted pharmacist license. 3. Education: Hold a Pharmacy degree. 4. Certification Exam: Pass the exam for BCMTMS Examination.;

Medical care services, namely, identifying and assessing medications taken by patients, providing consultation and advice to patients and caregivers regarding proper use of medications, consulting with patients, caregivers, physicians and other health care providers to develop and implement medication therapy programs for patients;BOARD CERTIFIED MEDICATION THERAPY MANAGEMENT SPECIALIST;The certification mark, as used or intended to be used by persons authorized by certifier, certifies or is intended to certify that the persons providing the services have met the educational, training and examination requirements needed for the skills and knowledge in providing services relating to medication therapy management.;
